Under the direction of the Transplant Social Work Supervisor, this unique position will support liver and kidney transplant recipients, donors and their families. Candidate will follow patients through all phases of transplant (pre-transplant psychosocial assessments, hospital stay and post-transplant follow up. Candidate may also provide coverage for the heart and lung programs as needed. Candidate will facilitate transplant and caregiver support groups and participate in various transplant committees.
Responsible for providing resources for local housing for out-of-town patients/families. Must Assist in coordination of discharge planning as needed and collaborate with multi-disciplinary team, transplant staff and social workers in other programs. Position includes crisis intervention, mandated reporting, information and referral coordination of substance use disorder and mental health services with community providers. The social worker is responsible for education and advocacy of patients, families and/or significant others. Candidate will participate in related department functions, such as community and program education. Performs all other duties as assigned.
Depending on the experience of the selected candidate, this position will be hired at a Clinical Social Worker II or Clinical Social Worker III level.
A Master's degree in social work from an accredited organization.
At least two (2) years of post MSW experience. In order to be considered for the Clinical Social Worker III, must have at least three years of post MSW experience and be a Licensed Clinical Social Worker (LCSW).

Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients, and external constituents, both verbally and in writing.
- Licensed Clinical Social Worker (LCSW) - issued by the state of California.
- Bilingual in both English and Spanish.
- Prior experience working in solid organ transplant.
- Experience working in both acute inpatient hospital and outpatient clinic settings.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
Pay Transparency Act
Annual Full Pay Range: $95,626 - $125,990 (will be prorated if the appointment percentage is less than 100%)
Hourly Equivalent: $45.80 - $60.34
Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
