The Staff Pharmacists in the Specialty Medication Management (SMM) Clinic are responsible for providing direct patient care to patients in various specialty populations at UCSD Health. The pharmacist in this position will conduct virtual and/or in-person visits with patients with endocrine and metabolic disorders, focusing on those patients on high-cost, high-risk specialty medications. The pharmacist will work under a Collaborative Practice Agreement to provide patient education and counseling, optimize therapeutic regimens, and address medication access issues, in collaboration with the clinic, pharmacy staff and interdisciplinary teams. The pharmacist will be responsible for maintaining a standardized medication use process and working closely with the UCSD Health Specialty Pharmacy and other pharmacies to ensure timely access to medications. Disease state areas of focus for this position encompass a wide range of endocrine and metabolic disorders including but not limited to diabetes, obesity, osteoporosis, and growth hormone disorders.
Roles and responsibilities:
- Conduct virtual 1:1 appointments with patients to assist in managing disease states such as diabetes, obesity, and other endocrine disorders
- Order labs and other health maintenance monitoring parameters
- Work with pharmacy technician team to support medication access and troubleshoot cost issues for patients, including appeals and peer to peers
- Assist in centralization of medication ordering processes, in particular around the use of GLP RA
- Provide medication reconciliation and counseling to patients
- Provide educational in-services to clinic staff
Depending on experience of the selected candidate, this position will be hired at the Stf Pharm1 or Stf Pharm2. Less than four (4) years experience qualifies as Stf Pharm1; Four (4) + years of experience qualifies as Stf Pharm2.
Doctorate of Pharmacy degree or equivalent clinical experience.
Registered Pharmacist - issued by the state of California (RPH).
Post graduate year 1 (PGY1) or equivalent clinical experience.
- Post graduate year 2 [PGY2] in area of specialty practice or equivalent experience in specialty area.
- Board Certification in Pharmacotherapy and/or Board Certification in Specialty Practice.
- Proficiency with Epic electronic health record system Experience in pharmacotherapeutic management, education and counseling of patients in desired specialty areas.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
Pay Transparency Act
Annual Full Pay Range: $174,390 - $196,356 (will be prorated if the appointment percentage is less than 100%)
Hourly Equivalent: $83.52 - $94.04
Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
