Cathedral Square Corporation, a nonprofit organization providing housing and services to our aging population in Vermont, is seeking a Human Services Coordinator to implement our SASH Services.

Summary:

The Human Services Coordinator (SASH Coordinator) will build trusting relationships with residents and community members participating in SASH and is the key communicator on site regarding the SASH model. The SASH Coordinator convenes regular meetings of the SASH team which includes designated staff members from community provider agencies to coordinate care and services for SASH participants. The SASH Coordinator’s day-to-day responsibilities fall into three broad categories of interventions that the SASH model combines and provides to SASH participants in a person-centered and holistic manner: coordinated care, self-management education and transitions support.

Qualifications:

Must possess a Bachelors Degree in Social Work or equivalent combination of background and experience. Previous experience coordinating services specifically for the senior or adult with disability client population is preferred. Knowledge of area resources and programs available to seniors and adults with disabilities, including transportation, health services, and recreational activities is essential. Demonstrated excellence in verbal and written communication is required. Must have knowledge of and an appreciation for the heritage, values, and wisdom of each resident and a commitment to the philosophy of a person’s choice to age at home. Must possess a valid driver’s license. A working knowledge of MSWord and Publisher is highly desirable. Pay is open to negotiate based on experience.

Hours and Location:

Whitney Hill (Williston, VT) 20 hrs, Cathedral Square Assisted Living( Downtown Burlington) 16 hrs, Richmond Terrace (Richmond, VT) 4 hrs.

Pay: From $18.00 per hour
